Introduction to Root Cause Analysis Part I


   Root cause analysis (RCA) is a structured, investigative approach used to identify and eliminate the root cause of a problem.

   By digging deep into the root causes of a problem rather than just addressing its symptoms, RCA aims to prevent the problem from returning in the future.


 Root Cause Analysis Goals

   ✅ Identify and Eliminate the Root Cause:  Determine and address the fundamental cause of a problem to resolve the issue at its source rather than just treating the symptoms.

   ✅ Prevent Recurrence: Implement countermeasures that prevent the problem from reoccurring, ensuring long-term stability and improvement.

   ✅ Remove Barriers to Work: Identify and eliminate obstacles that hinder employees' work, enabling smoother and more effective task execution.

  ✅ Deliver Exceptional Customer Value: Ensure that improvements address customer needs and contribute to delivering higher value and satisfaction.

   ✅ Develop Problem-Solving Skills: Equip team members with the skills and tools needed to effectively identify and solve problems, promoting a proactive approach.

  ✅ Improve Processes and Increase Productivity: Enhance operational processes by eliminating root causes, leading to more efficient operations and higher productivity.

 ✅ Standardize Problem-Solving:  Establish consistent methods for documenting, analyzing, and addressing problems to ensure uniformity and clarity in tackling issues.

 ✅ Foster a Culture of Problem-Solving and Sharing Best Practices: Encourage regular application of root cause analysis techniques within the organization, and promote the sharing of best practices to enhance collective learning and continuous improvement. 


Workshop Objectives

After participating in this workshop, attendees will be able to:

✅ Grasp RCA fundamentals: Understand the purpose and benefits of RCA.

✅ Identify organizational problems: Recognize different types of issues within an organization and determine which types require RCA.

✅ Apply Scientific thinking: Use DMAIC and A3 reports for structured problem-solving.

✅ Formulate clear problems statements: Develop and refine problems statements.

✅ Set target conditions using the SMART method: Establish effective targets using a proven approach.

✅ Master fundamental RCA tools:  Apply Fishbone diagrams, Pareto Analysis, and the Five Whys to a real problem.
