Workshop Objectives

1.     Reinforce key concepts from the Introduction to Root Cause Analysis Part I to ensure a solid foundation for Part II.

2.     Explore the purpose, different categories, and provide examples of effective countermeasures to address root causes and drive continuous improvement.

3.     Incorporate lean principles and methods into the root cause analysis process.

4.     Enhance creativity and problem-solving skills by practicing various techniques such as brainstorming, brainwriting, and CEDAC exercises to generate a diverse range of potential countermeasures.

5.     Utilize tools like Failure Mode and Effects Analysis (FMEA), Pareto analysis, and cost/benefit analysis to prioritize countermeasures effectively based on impact, feasibility, and risk.

6.     Implement decision-making techniques such as voting and building consensus to select the most suitable countermeasures for implementation.

7.     Develop A3 improvement plans that include each selected countermeasure.

  1. Icebreaker exercise.
  2. Review of introduction to root cause analysis part I.
  3. Countermeasures: purpose, categories, and examples.
  4. Incorporating lean principles and methods to address root causes and attain target conditions.
  5. Generating potential countermeasures through brainstorming, brainwriting, and CEDAC exercises.
  6. Prioritizing countermeasures with FMEA, Pareto, and cost/benefit analysis.
  7. Selecting countermeasures with voting techniques and consensus.
  8. Formulating and monitoring A3 improvement plans.
