What is Root Cause Analysis?


   Root cause analysis (RCA) is a structured, investigative approach used by individuals or teams to identify the underlying reasons or origins of a problem.

   By digging deep into the root causes of a problem rather than just addressing its symptoms, RCA aims to prevent the problem from returning in the future.

   This systematic approach involves following scientific thinking--gathering facts and data through direct observation, using quality tools to establish cause-and-effect relationships, and determining the the dominant causes of the problem through in-depth analysis. 

Workshop Outline

  1. Introduction to RCA: definition, purpose, benefits.
  2. Overview of scientific thinking, DMAIC, and A3 reports.
  3. Creating problem statements and and Is/Is not learning exercise.
  4. Collection of facts and data through directly observation.
  5.  Overview of preliminary RCA tools: Fishbone diagrams (categorize potential causes), Pareto charts (prioritize the most likely causes), and Five Whys (uncover root causes).
  6. Hands-on exercises using preliminary RCA tools.
  7. Run-down of additional RCA tools (Fault Trees, CEDAC, FMEA, and Voting.)
